Designing a Bathroom to Stay in for Thirty Years
Accessible design has a reputation for looking institutional. It has not been true for years. Almost all of it is structure and dimensions, decided at rough-in, invisible afterwards.
Two versions of this project exist. In the first, a bathroom is being remodeled anyway and a handful of decisions are made differently at almost no extra cost. In the second, someone has fallen, a hospital discharge date is fixed, and a bathroom that was finished four years ago is being opened up again.
The work is nearly identical. The difference in cost, disruption and stress is enormous. This guide is mostly an argument for the first version.
Why this is a rough-in decision
Almost everything that makes a bathroom work for someone with limited mobility is structural and dimensional rather than a product you can add later. Blocking inside walls. A recessed shower floor. A door opening width. Clear space beside a toilet. Circuits and boxes for better lighting.
All of that is decided in the week the walls are open. Grab bars, a shower seat, a comfort-height toilet and lever taps can all be fitted later; the wall that carries the grab bar and the floor that lets a wheeled shower chair roll in cannot.
Which produces a simple recommendation. If you are remodeling a bathroom in a house you intend to grow old in, build the structure for it now, even if you fit nothing.
The curbless entry
The single highest-value element. A four inch shower curb is a serious trip hazard for anyone with a walker, a cane or reduced foot clearance, and it is an absolute barrier to a wheeled shower chair.
A curbless shower has no threshold: the floor runs flat from the room into the shower, with the fall to the drain created within the shower area itself. A linear drain at the far edge is what makes it work with large-format tile, because the floor can fall in one plane.
It costs $3,000 to $6,000 more than a curbed shower, and more on a slab foundation because the concrete has to be cut and recessed rather than the pan dropped between joists. Since most Mountain View housing is slab on grade, that is the usual case here. It is still far cheaper now than as a retrofit.
Practical details that make a curbless shower work rather than flood: a generous shower area so spray does not reach the opening, a linear drain positioned at the far side, a slight fall across the whole wet zone, and a fixed glass panel that shields the opening without enclosing it.
Blocking, and why bars fall off walls
A grab bar is designed to take 250 pounds. Drywall anchors do not hold that reliably in a wet room over years, and a grab bar that comes off the wall when someone puts their weight on it is worse than no grab bar, because they were relying on it.
The correct method is timber blocking fixed between the studs while the walls are open, giving a solid backing anywhere a bar might go. While the walls are open this is a small amount of timber and an hour of labor. Afterwards it is a wall demolition.
So block generously. Every plausible location, not just the ones you intend to use: both walls of the shower, beside the toilet, and along any wall someone might steady themselves against. Nobody has ever regretted having too much timber inside a wall, and the marginal cost is close to zero.
Modern bars in brushed nickel, matte black or brushed brass are indistinguishable from towel rails, and several are designed to serve as both. The institutional look is a choice now, not a constraint.
Widths, heights and clearances
- Door opening. 32 inches clear is the working minimum; 36 inches is what a wheelchair needs. Note "clear", which is the actual opening with the door in place rather than the frame size. Where widening is impossible, a swing-clear hinge recovers a couple of useful inches.
- Toilet height. Comfort height, with a rim around 17 to 19 inches, is substantially easier to rise from than the traditional 15. It is now the more common specification anyway.
- Clear space. Room to approach and turn matters more than any individual fixture. A wheelchair needs a 60 inch turning circle, which many small bathrooms cannot provide, but even partial clearance beside the toilet and in front of the basin makes a practical difference.
- Basin knee clearance. A wall-hung basin or a vanity with an open underside allows a seated user to get close. It also happens to be the same wall-hung vanity that makes a small bathroom read larger.
- Shower seat. Built-in and structurally supported, or a fold-down unit on blocked wall. Roughly 17 to 19 inches high, like the toilet.
Floors, controls and light
Floor. A wet dynamic coefficient of friction of at least 0.42, and higher is better in a shower. Matte porcelain rather than polished. Smaller tile or mosaic on the shower floor adds grout lines, which adds texture underfoot.
Controls. Lever handles on doors and single-lever mixers on taps, both operable without gripping or twisting. A thermostatic shower valve with scald protection matters more as skin sensitivity declines. Position shower controls near the entry so the water can be started without stepping into the spray.
Light. Older eyes need considerably more light and are more sensitive to glare, so the answer is more output from more directions with good diffusion, rather than one bright fitting. A night light or motion-triggered low-level light on the route to the bathroom prevents more falls than most of the equipment in this guide.
Contrast. A quiet one that gets missed. A toilet seat, grab bar or basin edge that contrasts with the surface behind it is much easier to locate for anyone with reduced vision. All-white on all-white is elegant and hard to see.
Cost and funding
A full aging-in-place bathroom in Mountain View runs $30,000 to $70,000. As an addition to a remodel already happening, the incremental cost of blocking, a curbless entry, a wider door and better lighting is far smaller, generally $2,000 to $8,000 depending on how much of it is the curbless shower.
On funding: traditional Medicare does not cover home remodeling. Some Medicare Advantage plans include limited supplemental benefits covering specific safety items, and long-term care policies sometimes cover home modification with a physician recommendation. California's Medi-Cal waiver programmes can fund accessibility modification in some circumstances through a case manager. Remodelers do not bill any of these directly, so the route is an itemised invoice the family submits. Check the specific plan rather than assuming either way.
Where an occupational therapist is already working with the household, involving them at the design stage produces a bathroom fitted to the actual person rather than a generic accessible layout. It is worth an hour on site before anything is drawn.
